Pause and notice: What has felt hardest to carry during the past two weeks? Write a few words without trying to make them sound polished.
1. What is taking the most from you?
Constant worry, overthinking, tension, or expecting the worst
Grief, loss, or a change that has altered how life feels
Trauma responses such as shutting down, feeling on guard, or reliving the past
Low energy, disconnection, irritability, or difficulty finding meaning
Family, relationship, military-life, or substance-use concerns
Circle or write the concern that feels most urgent. You do not need to explain your whole history during a consultation.
2. What would feel different if support were helping?
Complete one sentence: “I would know therapy was helping if I could…”
Examples: sleep more steadily; respond instead of react; feel less alone in grief; understand my triggers; communicate without shutting down; feel more like myself.
3. Questions you can bring to a consultation
Does the therapist work with my concern and age group?
Is in-person or telehealth care available for my location?
Does the clinician accept my insurance or benefit program?
What happens after the consultation if the fit feels right?
